Dr. Sameer Ohri, M.D. MEDICAL DIRECTOR
Dr. Sameer Ohri, MD has over 20 years of experience in the medical field, and has been a Hospice Physician for decades. He is affiliated with medical facilities In Corona and Hospitals like Parkview Community Hospital Medical Center and Southwest Healthcare Corona Regional Medical Center.
Dr. Jason Wiguna DO, MBA ASSOCIATE MEDICAL DIRECTOR
Dr Wiguna is a hospitalist of Prime Hospital in Orange County, and also has an MBA from UCLA. He has a strong affiliation to OC Skilled Nursing Facilities. When not seeing patients, he enjoys spending time with his 3 kids in Disneyland or playing golf with colleagues and his wife, Dr. Tiffany Wiguna. Making steak is also one of his field of expertise!
Luis Rivas, BSN RN PHN DIRECTOR OF CLINICAL SERVICES & EXEC. DIRECTOR
Luis is a devoted family man, and dedicated husband, who brings over two decades of profound experience in hospice care to his role. With a comprehensive understanding of every facet of hospice operations, Luis has traversed various roles from his beginnings as a Certified Nursing Assistant (CNA),
progressing through Licensed Vocational Nurse (LVN), Registered Nurse (RN), Admission Director, Director of Patient Care Services (DPCS), all the way to Executive Director. His journey through these roles has not only honed his skills but also enriched his perspective, making him a valuable asset to any team. Beyond his professional commitments, Luis finds joy in spending quality time with his wife, often vacationing or immersing himself in the electric atmosphere of sports events. An avid golfer, he relishes the camaraderie of the green, frequently engaging in spirited rounds with colleagues and friends. Despite his demanding schedule, Luis always finds time to balance his passion for hospice care with his personal interests, embodying the essence of a well-rounded individual committed to both his profession and his happiness.

Ana Galvan, LVN ASSISTANT DIRECTOR OF PATIENT CARE SERVICES
Ana has been in the health care for the last 10 years, she started off as a CNA, and worked her way up to becoming an LVN. Ana was once a Wellness Nurse Director of a prestigious well-respected RCFE in North Orange County.
She started hospice as a field skilled nurse four years ago and with her keen attention to details she got her promoted as a QAPI Coordinator, to becoming Assistant Director of Patient Care Services.
She has two kids and when not with them, she spends her free time watching Sporting events, and going out to see movies with husband and friends.
Julie Muñoz - Lopez, LVN IE | SB Clinical liaison
Juliana has been an LVN for the past 16 years, she has worked as a School Nurse for Medically Fragile Children, Pediatric Home Health Nurse, Alzheimer''s Unit Nurse, Skilled Nursing Nurse and Case Manager, Marketing Director and Admissions Nurse.
For the past six years Juliana has been working in Hospice as a Clinical Liaison helping patients and families understand Hospice.
Juliana is also a certified CPR instructor. In her free time Juliana enjoys spending time with her four children from taking them to baseball and volleyball practices/games and traveling.
Daria Carrillo, LVN OC | LA CLINICAL LIAISON
Daria has been in the health care industry for over 30 years. Once a caregiver, medical assistant,Phlebotomist, Case Manager, Hospice Admissions coordinator, Hospice Regional Manager, SNF Director of Business Development.
With her lengthy years of health care experience now she has found her calling in helping families during the most difficult time of their lives. As a Clinical Liaison, she fell in-love being an advocate for families and patients.
Always eager to help and learn on the daily!She enjoys spending time with her 4 kids and playing board games with them during her down time.

Meet our Business Development Team
Mel Mendoza REGIONAL DIRECTOR OF BUSINESS DEVELOPMENT FOR OC & LA COUNTY
Mel, holds a position as a Director of Business Development for Orange County and LA County. She is responsible for the business expansion and strategic planning of Kind Heart Hospice. She has decades experience as a Senior Territory Manager for Abbott International. She graduated with Bachelors’ Degree in Nursing and sought further education on Health Care Business Management. After going through an overwhelming phase with the death of her father, she took it upon herself to help one family at a time by making that transition less traumatic and easy for them. She goes above and beyond in helping other people to get to spend their one last time with their loved ones. She is known for always making the wishes of the family or patient come to reality.
She enjoys travelling with her son and friends when not at work and playing golf with her colleagues.
Lupe Andrade, LVN REGIONAL DIRECTOR OF BUSINESS DEVELOPMENT FOR SB & IE COUNTY
Lupe has been in Hospice for a total of 20 years. She started as an Intake coordinator. Her strong work ethics and bright personality made her to be hand picked as a Business Development for an upcoming Hospice agency at that time, and became the best at it. She made a big contributions to majority of patient’s and families lives and found her desire to continuously help people during their difficult times. Lupe enjoys being with her friends on the weekend and travels the world whenever her schedule allows with her son.
